what are the easy ways to add advertising on a forum? let's say SMF.
simple ways to include ads on pages,
and/or
simple to "get" (like signing up for googles ads is lots easier than contacting individual retailers and negotiating lots of little deals).
would consider google ads, banner ads, amazon ads, text links, etc, .......

For the first one, I was a big fan of phpAdsNew, which is apparently now called Openads - http://www.openads.org/products/overview.html.

I second that. I use PhpAdsNew (as said now OpenAds) on 3 out of the 15+ forums that I run now. Very easy to setup and use.. definitely worth it!

I've heard of that software.
with such a common need (to run ads), why don't the major forum softwares have something like that built in?
where do you get your advertisers? could a commission junction
account provide the banners and ads/advertisers that would run under openads?

I'm not sure why the major forum softwares don't have some type of ad control built in.
But they all seem to have plugins available. :)
You get advertisers by supplying an easy way for visitors to become advertisers, or using google adwords, commission junction, or your preferred advertising leads generator. OpenAds makes it easy to integrate these types of ads.

When advertising forums you should market on directories and other forums as well, telling the users what do you offer on your community and also differentiated what kind of discussion you are talking about.... advertising forum takes time , hope this help, good luck.
